Actually one of the least allergenic fish. The main risk is scombroid: histamine from poor refrigeration, not true allergy. If reactions happen sometimes but not always, a badly stored batch is the likely cause.

Reaction Timeline
Scombroid symptoms (flushing, hives, GI distress) appear within 10–60 minutes. True IgE fish allergy is also immediate. Dermatologic manifestations occur in 82.2% of scombroid cases — it can look exactly like an allergic eczema flare.


How Much Is Needed To React?
Dose-dependent
For scombroid: the histamine level depends entirely on storage conditions, not portion size. A small piece of improperly stored tuna can contain enormous histamine levels. For true fish allergy (rare with tuna): reactions may occur at low doses. Buy from reputable sources and ensure continuous cold chain.
Does Preparation Matter?
Yes — preparation significantly changes reactivity
Fresh tuna stored properly at <4°C has minimal histamine. Histamine forms during improper storage and is NOT destroyed by cooking, canning, or freezing. Once formed, histamine is permanent. Sashimi-grade tuna (flash-frozen at sea) typically has low histamine. Canned tuna has variable histamine depending on the raw material quality before canning. [3][4]


Also Watch Out For...
Other Scombridae (mackerel, bonito, skipjack) — same scombroid risk [4]
Cod, salmon, herring — LOW cross-reactivity because tuna has very little parvalbumin [3]
Shellfish — NO cross-reactivity between finfish and shellfish
What To Use Instead
Salmon (if fish allergy is not the issue — higher parvalbumin but good omega-3s; note: salmon is on trigger list)
Chicken breast (for salads and sandwiches in place of tuna salad)
Chickpea "tuna" salad (note: chickpeas on trigger list)
Sardines may have similar scombroid risk — choose based on your specific issue
