Reaction Timeline
Gly m 4 reactions (birch cross-reactive) tend to be immediate and oral (OAS). Gly m 5/6 reactions can be immediate or delayed. AD flares from soy are often delayed 12–48 hours.


How Much Is Needed To React?
Any amount
Individuals with Gly m 5/6 sensitization may react to trace amounts in any soy form. Birch-cross-reactive individuals (Gly m 4) may tolerate cooked/fermented soy but react to soy milk specifically. Testing should distinguish which pathway applies.
Does Preparation Matter?
Yes — preparation significantly changes reactivity
This is critical: Gly m 4 is heat-labile, so fermented/cooked soy (soy sauce, miso, tempeh, tofu) is often tolerated by birch-pollen-related soy-allergic patients. Gly m 5/6 are heat-stable, so patients sensitized to these react to ALL forms. Soy milk is the worst-tolerated form. Soy sauce and miso are the best-tolerated. [25]

Birch pollen — strong cross-reactivity via Gly m 4/Bet v 1 (Spearman ~0.98) [25]
Peanut — cross-reactive via shared vicilin (Ara h 1/Gly m 5) and legumin (Ara h 3/Gly m 6) families
Other legumes (lentils, chickpeas) — variable cross-reactivity via shared storage proteins
What To Use Instead
Oat milk (for coffee, cereal — note: oats are on the trigger list)
Rice milk (mild flavor, widely available)
Coconut milk (note: coconut is on the trigger list)
Hemp milk (low allergenicity, good omega profile)

Soy lecithin in chocolate and processed foods
Soybean oil (usually refined and tolerated, but cold-pressed is not)
Vegetable protein in processed meats
Soy flour in baked goods
Infant formula (soy-based)
Asian sauces (teriyaki, hoisin)
Edamame in frozen vegetable mixes
Soy-based "meat" alternatives (Beyond, Impossible contain soy)
Miso in soup bases
Tempeh in vegetarian dishes
